To register your child in St. Anne's CYO Girls Basketball Program, please download and complete:

Note: You will need Acrobat Reader installed to view and print the forms and map.

Along with the above forms, you need to enclose a photo copy of your child's birth certificate and a copy of a current utility bill for each child you are going to register (home phone bill, PG&E bill, or garbage bill). We will not accept a copy of your drivers license, cell phone bill, or bank/mortgage statements.

All forms must be filled out completely, you must provide us with the birth certificate and utility bill for each child. Girls will be registered on the team when the registration forms are received, if the forms have not been filled out completely, or you haven't provided us with the supporting documents that we need, than your child will not be registered until you provide us with the balance of the information/documents that we need.

Mail the forms to the address indicated on page 2 of the forms document.


About CYO Girls Basketball: CYO Girls Basketball is open to all girls who are in St. Anne's CCD Religious Education program or girls that live within St. Anne's Parish boundaries (West of Hwy. 880, North of the Alameda Creek, by Lowry Rd. in Fremont/Union City, and up to the Union City/Hayward border), regardless of religious faith. Girls that are enrolled in St. Anne's CCD program do receive preference in sign-ups, and as members on a team.

This program is run in grades 3 - 8, but we do allow girls in grades 2 to sign up and they would play on the 3rd grade team. Girls must register for the team/grade, as the grade they are in school this year.


Cost/Fees to Play: First child $75.00, additional children $70.00.


Uniforms: If your child is new to our program, or needs a new uniform to replace last years uniform, the cost of the uniforms are $45.00 for adult sizes, and $40.00 for youth sizes ( you will own these uniforms, and they may be worn for as long as they fit ). If you would like to purchase a T-shirt for under your child's jersey, you may purchase one from us for $5.00. We have set up a date at St. Anne's Parish for those who would like to check on the size of the uniform from our sample size uniforms, the date for that is Monday, January 16, 2006 from 7pm - 9pm in room 101, or you may contact Debbie and make arrangements.


Practices: We hold our practices at Alvarado Middle School in the gym. We will start our practices the first week of February, in Feb. the boy's basketball program will still be going on, but starting to wrap up, as the boy's team's complete their season, we will be moving the girl's teams into their permanent days and times for practices, but we may need to use different days and times at first, until the boys finish. Once we get settled, the girl's normal practice days and times will be; girls in grades 3,4,5, Tue. and Thurs. from 6-730pm, and girls grades 6,7,8, will be Tue. and Thurs. from 7:30 - 9 pm.
