for OCTOBER 2006 – MAY 2007 School Year The information sheet and registration form for the 2006-2007 School of Faith Formation (CCD) Registration program year are now available for download in Word document format. Both documents are also available in one pdf file. In order to participate in the religious education program of our School of Faith & Formation, - Each family must be registered in the parish and financially support the parish through the Sunday collection. - Please submit a copy of child’s Baptismal Certificate at time of registration (for new students only). New Students The new tuition fees for the school year 2006-2007 are listed below. These tuition fees pay for textbooks, supplies, program expenses, administrative costs and catechist training. We appreciate payment of tuition at the time of registration. Please make checks payable to ST. ANNE CHURCH. $ 95.00 for one (1) child $110.00 for two (2) children per family $125.00 for three (3) children per family $160.00 for four (4) children per family $175.00 for five (5) or more children per family Registration for NEW STUDENTS will be accepted on the following dates: JULY Saturday, July 15 9:00am - 12:00pm Thursday, July 20 4:00pm - 7:30pm AUGUST Saturday, Aug 12 9:00am - 12:00pm Monday, Aug 14 4:00pm - 7:30pm Thursday, Aug 17 4:00pm - 7:30pm SEPTEMBER Final Registration Week Mon – Thurs, Sept 11-14 4:00pm - 7:30pm Saturday, Sept 17 9:00am – 12:00pm Returning Students Parent/Guardian of RETURNING STUDENTS - Please read this registration packet carefully for there have been changes to the program for 2006-2007 school year. Kindly return/mail completed registration form with payment to the Faith Formation Office by July 8, 2006. Registration will be on a first come served basis. In September, a schedule letter confirming time and date of classes will be mailed to those families who have registered in June, July & August.
