*If your child has been baptized in a parish other than Corpus Christi, a copy of the certificate is needed for the parish files. This will allow the processing of documents related to the sacraments to proceed with ease.  

Once you have filled out this request, arrangements will be made for you to sign an official document.

A new fee structure is in place for the 2011-2012 year:
Faith First fees are $70.00 1st child, $60.00 2nd child, $35.00 3rd child.

Note: There will be a $25.00 late fee applied, per family, for all registrations received after July 1st.
