Permission Slips are required for activities that take place off of parish grounds. Those under the age of 18
     must have a Permission Slip for all these activities. Simply click on the desired form, print it and completely
     fill it out. Return it to the Youth Ministry Office before the date of the activity. (Activities during the month
     of January do not require any permission slips).
