FAITH FORMATION REGISTRATION AND FEE INFORMATION
Only registered participating parishioners of OLA may be accepted into our parish Faith Formation or Sacramental Preparation program.
Status of parish membership must be ACTIVE (questions concerning membership status may be directed to the parish office).
Families not practicing stewardship through weekly parish envelope contributions must have the pastor's approval for sacramental preparation registration.
Copies of baptismal certificates must accompany registration.
Children must be toilet-trained for enrollment in our Pre-K program (age 4).
Public School students are required to receive their religious instruction through our parish faith formation program.
Faith Formation enrollment of public school students is expected beginning in Kindergarten and continuing through Grade 12.
Students in Grades 9-12 who have been confirmed are expected to register in our parish Senior Youth Group for continuing catechesis.
Students in Grade 3 and above with no previous religious instruction who have not received the Sacraments of Reconciliation and Eucharist will be placed in our Youth Development (Special Sacrament) Class.
FAITH FORMATION FEES: $25 PER CHILD. FEES ARE DUE AT THE TIME OF REGISTRATION

SACRAMENTAL PREPARATION REGISTRATION AND FEE INFORMATION
FIRST RECONCILIATION AND FIRST EUCHARIST
FAMILIES MUST SUBMIT A COPY OF THEIR CHILD'S BAPTISMAL CERTIFICATE FOR SACRAMENTAL REGISTRATION
Public School Students are required to attend Faith Formation classes with excellent attendance in Grade 1 to be eligible for First Reconciliation and First Eucharist in Grade 2.
Public School Students enrolled in Grade 2 Faith Formation (who completed Grade 1 FF) are automatically enrolled.
Public School Families whose children attend public school with enrollment in the parish faith formation and sacramental preparation program pay both fees.
Catholic School Students
Catholic School Students register for Sacramental Preparation Only in Grade 2.
Catholic School Families registering for Sacramental Preparation must complete a Faith Formation Registration Form.
Catholic School Families pay only the Sacramental Fee.
SACRAMENTAL FEES FOR FIRST RECONCILIATION AND FIRST EUCHARIST: $35 PER CHILD
SACRAMENTAL INFORMATION WILL BE MAILED TO REGISTERED FAMILIES IN FALL 2003.

CONFIRMATION
FAMILIES MUST SUBMIT A COPY OF THEIR CHILD'S BAPTISMAL CERTIFICATE FOR SACRAMENTAL REGISTRATION
Public School Students are required to attend Faith Formation classes with excellent attendance in both Grades 7 and 8 to be eligible for Confirmation.
Catholic School Students register for Confirmation in Grade 8 ONLY.
Catholic School Families registering for Confirmation must complete a Faith Formation Registration Form.
SACRAMENTAL FEE FOR CONFIRMATION: $45 PER CHILD
SACRAMENTAL INFORMATION WILL BE MAILED TO REGISTERED FAMILIES IN WINTER 2004.

REGISTRATION/FORMS
Registration into our parish Faith Formation and Sacramental Preparation programs may be completed through the Faith Formation Office during normal business hours or by appointment, mail or drop off.
Registration Forms are available at the Faith Formation bulletin boards (wall bin) located in the church vestibule.

Faith Formation Classes: Sundays (those scheduled) 9:45-10:45AM OLA School (FF Handbooks will be available Fall 2003)
Youth Development: Tuesdays (those scheduled) 6:30-8:00 PM Church
First Reconciliation and First Eucharist: NO CLASS TIME
